Step 1 - Initial Review

Call, text, or fill out the form linked below. We’ll reach out to answer your questions before moving forward to Step 2.

Step 2 - Complete Required Forms

We’ll email you a form to gather information about your SAP, your violation, and basic demographic details. Once completed, return it to us along with the $175 fee.

Step 3 - SAP Eligibility Verification

We will request R2D eligibility documentation directly from your Substance Abuse Professional. Once your eligibility is verified, we will move to Step 4.

Step 4 - Test Instructions

You will be provided with testing instructions, including how to designate DOTDOC as your C/TPA in the FMCSA Clearinghouse and where to complete your return-to-duty test.

Step 5 - Testing

At this stage, you can complete either your DOT look-alike test or your official observed return-to-duty drug and/or breath alcohol test at an approved testing site.

Step 6 - Results

When we receive your negative test result, we will update your Clearinghouse status and complete Step 5 — making you eligible to operate a CMV again.

Frequently Asked Questions

Do I Need An Employer To Complete Step 5?
  • No, an employer is not required for Step 5. You can still complete your return-to-duty testing even if you are not currently employed or are self-employed.
  • In most cases, we can schedule your return-to-duty the same week you reach out. Timing depends on how quickly we receive your SAP paperwork.
  • DOT look-alike tests can be schedule same-day.
  • You can’t take a practice return-to-duty (R2D) test, but you can take a DOT look-alike test that mirrors the regulated test. It screens for the same substances at the same cutoff levels. If you test negative, you can move forward with confidence. If you test positive, it does not count against you.
  • After we receive the negative R2D results, we report them to the Clearinghouse.
  • Immediately after submission, your status goes from “Prohibited” to “Not Prohibited”.
  • You can then contact your local DMV/SDLA to confirm that your privileges have been reinstated.
  • If you fail your test and believe it was an error, you can request a retest through the MRO (Medical Review Officer). This typically costs $200.
  • A failed test will also result in a new Clearinghouse violation, meaning you will have to restart the process from Step 1.
  • If you have any doubts about passing, ask us about a practice DOT drug test before scheduling your observed test.

  • The $175 fee includes:
    • Observed Return-to-Duty drug test OR Breath Alcohol Test
    • Clearinghouse reporting of results
    • Guidance through the process
  • If you need BOTH drug and alcohol testing, there will be an additional $75 fee
  • Absolutely. We serve drivers across the U.S., providing nationwide C/TPA services and scheduling your Step 5 test at a qualified location near you.

We created this service because we saw how frustrating the process can be. Too often, there’s little accountability or real support, leaving drivers to figure it out on their own. We believe drivers deserve clear guidance, responsive communication, and a process done right from start to finish. When you work with us, you’re working with real people that are accessible, involved, and focused on getting you back to work.
